Key nursing diagnoses and patient outcome

Impaired physical mobility related to muscle rigidity and spasms.Based on this nursing diagnosis, you will establish these patient outcome.The patient will

Show no evidence of complication , such as contractures ,venous stasis ,thrombus formation ,or skin breakdown during the period of immobility

Regain normal physical mobility with eradication of tetanus

Ineffective airway clearance related to rigid neck muscles .based on this nursing diagnosis, you will establish these patient outcome. The patient will
Maintain a patient airway

Not have dyspnea or a change in respiratory pattern

Maintain adequate ventilation , as evidenced by clear breath sound and normal arterial blood gas value

Acute pain related to muscle rigidity and spasms. Based on this nursing diagnosis ,you will establish these patient outcome . the patient will
Identify adequate activities and body movements that precipitate or increase pain and make nurses aware of them

Adhere to activity restriction to decrease or prevent painful muscle spasms

Report pain relief following administration of a muscle relaxant

Nursing interventions
Before tetanus develops, thoroughly debride and clan the injury site with 3% hydrogen peroxide and check the patient's immunization history .record the cause of the injury. If it was caused by an animal bite, report the case to local public health authorities

Before giving penicillin and tetanus immune globulin, antitoxin, or toxoid, obtain an accurate history of the patient's allergies to immunizations or penicillin. If the patient has a history of any allergies, keep epinephrine 1:1,000(for subcutaneous
Injection )and emergency airway equipment available

After tetanus develops, maintain an adequate airway and ventilation to prevent pneumonia and atelectasis. Suction as needed

Insert an artificial airway, if necessary, to prevent tongue injury, and maintain the airway during spasms

Keep emergency airway equipment on hand because the patient may require artificial ventilation or oxygen administration. Have endotracheal and tracheotomy equipment on hand. In an emergency, the doctor may perform tracheotomy if the patient become extremity rigid

Be prepared to resuscitate the patient and initiate life support

Administer I.V. therapy as prescribed

Because even minimal external stimulation provokes muscle spasms, keep the patient's room dark and quit

Turn the patient frequently to prevent contracture, pressure ulcer and pulmonary stasis

Perform all activities of daily living for the sedated patient

If urine retention developa, insert an indwelling urinary catheter

Provide adequate nutrition to meet the patient's increased metabolic needs. He may need nasogastric feeding or total parenterl nutrition


Monitoring

Watch for sign of respiratory distress

Monitor intake and output

Frequently check the patient's ECG for arrhythmias monitor the patient's nutrition status

Assess the patient for increase muscle spasms and rigidity and for the effectiveness of administered muscle relaxants
